If you are applying for a mailbox by email or by phone, we will need to have a good copy of your valid, government-issued photo IDs (2). If you will be coming into the store at some point to pick up your mail, disregard this message. (This only applies to you if your are applying from out of the area and will want your mail forwarded to you.) Applying for a mailbox can be accomplished without even coming into the store. Here's how: 1. Request a faxed copy of the Mailbox application, or print it here. 2. Complete the Application. Box 2: Put the name of the person opening the box, including the business name. Box 5: Names of other people who will be receiving mail here. (Note: we will need to have a photocopy of each person's ID on file before we can release any mail to them -- per the USPS Regulations.) Box 6: Put your name here Box 7: Home address of the person opening the box. Box 9: Name of business (if applicable) Box 10: Address of business (if applicable) and Work Phone number (all applicants). Box 11: Kind of business (for business boxes only). Box 12: Names of people from the business who will be receiving mail here. Box 13: Names and addresses of Corporate officers. Box 14: Date, County, and State of Corporate registration. Box 15: Have the notary sign here ONLY if you cannot come into our store to open your box. Box 16: Signature of person opening box (box 7). For pages 2-4: Fill out all fields, initial where indicated, and then sign on the last page. 3. Have you signature on page one Notarized. Consult your local Yellow pages for Notaries in your area. 4. Make a clear copy of your Photo identification -- See Below -- and fax it and your completed application to 415.765.1565. If you are applying for a mailbox by email or by phone, we will need to have a good copy of your valid, government-issued photo ID. We will then contact you regarding payment (via credit card) and assignment of a mailbox number. |
